Orthodontic Case Giveaway

Do you know an outstanding person in our community who would benefit from braces but can’t afford it?
Starting on May 1st, you can nominate an individual for their chance to receive a FREE Orthodontic case!
To Nominate:
Make a Nomination Video or Photo Submission!
- Share their story and why they deserve a free orthodontic treatment.
- Video should be no longer than 2 minutes. The video will be reviewed for content and appropriateness before being posted to Facebook. The deadline for video or photo submission is September 1st.
- Be creative!
Email the Video/Photo to Jillian.summitfd@gmail.com
- Include the nominee’s and your contact information in the email (Full name, phone number and email address). Nominee’s personal information will not be shared publicly.
Your Video/Photo will be Posted to our Facebook

Selection:
Winner will be selected by a panel of employees and will be weighted equally based on:Social Media Followers’ Choice
- The video/photo that is shared and liked most by followers
Financial Need
- Candidate must not have any other options for treatment (i.e. insurance, flex account)
Nominee’s StoryΒ
- Is there a hardship, illness, or severe self-esteem issue? Is the person actively giving back to the community?
The Winner Will Be Announced October 1st.
